Secondary transition refers to activities meant to prepare students with disabilities for movement from school to post-school activities. Statements about the student's transition needs must be included in the IEP after the student reaches a certain age. Annual Performance Report Indicator - Percent of youth aged 16 and above with an IEP that includes coordinated, measurable, annual IEP goals and transition services that will reasonably enable the child to meet the post-secondary goals.
